Ordering an Indiana Background Check in Person or by Mail. Limited criminal background checks in Indiana can also be obtained by delivering a paper application to the Indianapolis headquarters of the ISP. In addition to specific details about the subject of the record, applicants will have to elaborate on the purpose of the search as well.
Driver Record
A driver record contains your driving history. The record shows current and resolved court-ordered suspensions, citations, violations and other entries impacting your record - including reinstatement fees owed to the BMV.
Please Note: Outstanding issues with a court must be settled with the court, including failure to pay tickets, unsatisfied judgments and delinquent child support. The BMV cannot address court issues.
Two types of driving records are available.
Viewable Driver Record (VDR) - Can be viewed at no charge.
Official Driver Record (ODR) – A certified copy of your driver record that is accompanied by a letter of certification from the BMV. Some employers and school admission offices require a certified driver record. The ODR is available for $4.

If you do not know your driver’s license number, you will be required to request a copy of your Official Driver Record by mail. To order your ODR by U.S. Mail, print and mail the form below to the BMV. Remember to include your payment.
