In Utah, you are required to carry a driver's license in order to operate a motor vehicle. If you lose your driver's license, it's stolen, or you damage it to the point that critical information (like your name or date of birth) is illegible, you'll need to request a replacement driver's license from the Utah Department of Public Safety (DPS).

Apply for a Duplicate UT Driver's License

NOTE: You can renew your UT driver's license instead of applying for a duplicate if it expires within 6 months.

Since Utah does not require you to file a police report if your driver's license is lost, stolen, or damaged, it's not difficult to replace a lost UT driver's license―you will just need to appear in person at your local DPS office to apply for a duplicate.

You will need:

  • A completedDriver’s License Application (Form DLD6a).
  • 1 document proving your identity, such as a:
    • U.S. passport.
    • U.S. birth certificate (original or a certified copy).
    • Valid permanent resident card.
  • 2 documents to prove you are a UT resident, such as a:
    • Bank statement.
    • Property tax notice.
    • Court document.
  • 1 document to verify your Social Security number (SSN), such as:
    • Your Social Security card.
    • A W-2 form.
    • A SSA-1099 form.

Once you're at the UT DPS office, you will need to:

  • Have a new driver's license photo taken.
  • Pass the vision test.
  • Pay the $23 fee for a duplicate driver's license.

Your new Utah driver's license will expire on the same date as the license you replaced.

NOTE: If you're ordering a duplicate Driving Privilege Card, you may need to provide a fingerprint card and a photo of yourself in a sealed envelope.

Replace Your License While Out of State

If you misplace your UT driver's license or it is stolen while you are traveling out of state or abroad, you're out of luck until you return to Utah. The UT DPS requires you to appear inperson at a local driver license field office with the proper forms of identification in order to get a duplicate.

You might want to bring photocopies of your driver's license with you when traveling and keep these somewhere safe―in case of emergency.

Report a Missing Utah Driver's License

If you believe your driver's license was stolen, you aren't required to report it, but you may want to let the local Utah authorities know. Since identity theft is such a far-reaching problem these days, you might also consider reporting the lost or stolen license to the U.S. embassy if you're outside of the country.

Replace a Lost Motorcycle License

To replace your lost motorcycle license, you'll simply need to follow the same instructions listed above in ' Apply for a Duplicate UT Driver's License.'
